When clipped near the scalp, hair offers roughly a three-month detection timeline for substances. It is straightforward to collect, challenging to tamper with, and convenient to transport.

A 1.5-inch section consisting of approximately 200 hairs (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg, ideal for both preliminary and thorough analysis. For EtG, supplemental evaluations, or panels exceeding ten categories, 150mg is advisable. It is suggested to measure the sample on a jeweler’s scale. If head hair is lacking, a similar quantity of body hair is acceptable. 'Head hair' pertains solely to scalp hair, whereas 'body hair' covers all other types (facial, axillary, etc.).

Process Overview

The quartet of primary steps in the lab analysis of a drug test result covers Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample inside a laboratory system, including verification of proper sealing and shipping, assigning an arbitrary LAN (Laboratory Accessioning Number), and finalizing any extra data entry absent from an electronic chain of custody system.

Screening serves as an expedient initial evaluation for drug misuse. Although Screening efficiently excludes drug use in most samples, a confirmed screening is required for legal admissibility. All samples showing a presumptive positive in Screening necessitate subsequent confirmation.

For a sample deemed presumptively positive during Screening, additional hair is extracted from the original sample and prepped for Extraction, wherein drugs are retrieved from hair at significantly lower concentrations than other methods (e.g., urine or saliva), rendering hair drug screening the most complex method to execute.

The Confirmation of any positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing prior to confirmation if necessary. The comprehensive lab process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair classification and the certification to ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
