What it is, How it works

When hair is gathered close to the scalp, it can reveal a detection window of up to 3 months for alcohol and drug use. The process of collecting hair is straightforward, somewhat resistant to tampering, and easy to transport.

Gathering a 1.5-inch piece consisting of approximately 200 hair strands (or the diameter of a #2 pencil) nearest to the scalp results in 100mg of hair, which is the perfect sample size for both screening and confirmation tests. For EtG add-ons and tests exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair can't be collected, a comparable amount of body hair can be used. When addressing head hair, we mean only the scalp hair. Body hair includes all other types (such as facial, axillary, etc.).

Process Overview

There are four primary phases in the lab processing of a drug test result, namely Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is integrated into a lab's system. This step confirms the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any other data entry needed that is not already covered by an electronic chain of custody system.

Screening comprises a preliminary rapid assessment for drug abuse. While Screening provides a cost-effective means to exclude drug use in most samples, a positive result requires confirmation to be valid in court. Any samples that are tentatively positive during Screening require subsequent confirmation.

If a sample is deemed tentatively positive during Screening, additional hair is taken from the original sample and prepped for Extraction. This stage involves drugs being extracted from hair at considerably lower concentrations than other techniques (such as urine or oral fluid), which illustrates why hair drug screening is considered the most challenging methodology.

Positive screening findings are confirmed through GC/MS, GC/MS/MS or LC/MS/MS techniques. Prior to confirmation, all samples with tentatively positive results are washed if necessary. The entire lab process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair guideline and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
