What it is, How it works

Hair testing is recognized as a potent means for identifying drug and alcohol consumption. The hair captures a historical record of substances by embedding biomarkers within the hair strand as it grows. When obtained close to the scalp, hair offers a detection window of up to roughly three months for alcohol and other drugs. Hair collection is straightforward, relatively resistant to tampering, and easy to transport.

A specimen with a length of 1.5 inches consisting of approximately 200 hair strands (equivalent to a #2 pencil's thickness) closest to the scalp delivers 100mg of hair, the ideal quantity for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advised. We suggest assessing the specimen's weight with a jeweler’s scale. If scalp hair is absent, a similar amount of body hair may be obtained. Whenever head hair is mentioned, it refers specifically to scalp hair. Body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The four key phases in the lab analysis of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed within a lab’s system. This step ensures the sample is sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any needed data entry not covered by an electronic chain of custody system.

Screening involves a rapid preliminary assessment for drugs of abuse. While it's a budget-friendly approach to eliminate drug usage in the majority of samples, a positive screen must be verified for legal admissibility. Any samples showing presumptive positives during Screening need a secondary verification.

Should the sample be presumptively positive during the Screening phase, additional hair is taken from the original specimen and made ready for Extraction. Here, drugs are extracted from hair at a significantly lower concentration than in other methods (such as urine or oral fluid), making hair drug testing one of the most challenging methods to execute.

GC/MS, GC/MS/MS or LC/MS/MS is used to confirm any positive screening results. All presumptive positive samples undergo washing before confirmation when necessary. The entire laboratory process, from Accessioning through Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
