When conducting EtG, additional panels, or tests exceeding 10 panels, it's advisable to use 150mg of the sample. Utilizing a jeweler’s scale is recommended for weighing the specimen. If scalp hair cannot be obtained, an equivalent quantity of body hair can be used. By head hair, we mean scalp hair exclusively. Body hair encompasses all other kinds (such as facial and axillary).

Procedure Summary

The sample processing in a laboratory for a drug test result involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial entry of a sample into a lab’s system. The steps include ensuring the sample’s proper sealing and shipping, allocating a random LAN (Laboratory Accessioning Number), and finalizing any supplemental data that an electronic chain of custody system doesn't supply.

Screening provides an initial and quick assessment for substance misuse. Although a cost-effective method to eliminate drug presence in most samples, a positive result in Screening requires further confirmation for legal validation. Samples showing a positive result in this stage require additional verification.

Should a test be positive initially in Screening, more hair is extracted from the sample for the Extraction phase. During this phase, substances are isolated from hair in concentrations lower than other methods (like urine or oral fluid), underscoring the complexity of hair drug screening procedures.

Any positive result in Screening is verified via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo a wash as needed before confirmation. The complete lab process from Accessioning to Confirmation is overseen following both CAP (College of American Pathologists) Hair certification and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
