For EtG, additional testing, and/or assessments exceeding 10 panels, a 150mg specimen is advisable. It's best to weigh the sample using a jeweler’s scale. If scalp hair cannot be obtained, an equivalent quantity of body hair can be used. When discussing head hair, we specifically mean scalp hair. Body hair pertains to all other hair types such as facial, axillary, etc.

Process Overview

The primary steps in processing a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial procedure of integrating a sample into a lab’s system. This step entails ensuring the sample was correctly sealed and dispatched, allocating a unique LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not covered by an electronic chain of custody system.

Screening serves as a preliminary evaluation for the presence of abused substances. Although Screening offers a cost-efficient approach to exclude drug use in most instances, a positive screening result necessitates confirmation to become credible in legal situations. Samples yielding a preliminary positive result require further validation.

If a sample shows a presumptive positive during Screening, a portion of the original sample is prepared for Extraction. This phase involves isolating drugs from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting why hair drug testing is the most challenging technique to execute.

Any positive screening outcome is confirmed through GC/MS, GC/MS/MS or LC/MS/MS methodologies. All samples showing presumptive positive results undergo a washing process prior to confirmation. The comprehensive lab process from Accessioning through to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair category and adheres to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
