Hair analysis is now recognized as a significant method for identifying drug and alcohol consumption. Hair captures a prolonged account of substance intake by locking in biomarkers within the fibers of the growing hair strand. When taken close to the scalp, hair offers up to a roughly 3-month detection timeline for alcohol and various substances. Hair collection is straightforward, relatively tamper-resistant, and simple to transport.
A 1.5-inch sample consisting of around 200 hair strands (approximately the thickness of a #2 pencil) near the scalp provides 100mg of hair, which is the optimal sample size for testing and verification. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the hair sample is suggested. Measuring the specimen with a jeweler’s scale is recommended. If scalp hair isn't available, an equivalent quantity of body hair can be used. By "head hair," we exclusively mean scalp hair. "Body hair" includes all other hair types (facial, axillary, etc.).
Process Overview
The laboratory processing of a drug test result involves four main stages: Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the initial step of incorporating a sample into a lab's system. This process includes checking that the sample was properly sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data input not included by an electronic chain of custody system.
Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is an economical approach to exclude drug use in most samples, a positive screen must be verified for it to be valid in legal settings. Any samples with preliminary positive results during Screening require further confirmation.
If a sample tentatively shows positive results at the Screening phase, additional hair is extracted from the original sample and made ready for Extraction. During Extraction, drugs are isolated from hair at much lower concentrations compared to other methods (such as urine or oral fluid), which is why hair drug analysis is the most challenging method to execute.
Verification of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that initially test positive are cleansed prior to verification as necessary. The complete lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.
