If scalp hair cannot be sourced, an equivalent amount of body hair may be obtained. Head hair refers exclusively to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test consists of four fundamental steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails integrating a sample into a lab's system. This step verifies proper sealing and shipping, assigns a unique LAN (Laboratory Accessioning Number), and completes any required data entry absent from an electronic chain of custody system.

Samples that show a presumptive positive result in Screening require additional confirmation.

If Screening suggests a positive result, further hair is taken from the original sample and prepared for Extraction. This phase involves extracting drugs from hair at concentrations lower than those found in other testing methods (e.g., urine or oral fluid), underpinning the complexity of hair drug screening.

Confirmation of positive Screening outcomes uses GC/MS, GC/MS/MS, or LC/MS/MS techniques. Any presumptive positives undergo washing before confirmation if necessary. The laboratory process, from Accessioning to Confirmation, is conducted under both the CAP (College of American Pathologists) Hair proficiency and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
