What it is, How it works

Hair analysis is recognized as a highly effective method for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and drugs by embedding biomarkers within the hair's structure. If gathered near the scalp, hair can offer a detection window of up to around 3 months for alcohol and drugs. Collecting hair is straightforward, somewhat resistant to tampering, and simple to transport.

A hair sample measuring 1.5 inches and consisting of roughly 200 strands (about the thickness of a #2 pencil) nearest to the scalp will yield 100mg, which is ideal for both screening and validation. For EtG, additional tests, or those exceeding 10 panels, 150mg is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair isn't an option, an equivalent quantity of body hair can be used. By head hair, we mean only scalp hair. Body hair includes all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial processing of a sample within a laboratory’s framework. This process involves ensuring the sample was properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any data input not available via an electronic chain of custody.

Screening offers an initial assessment for the presence of drug abuse. While Screening offers a cost-efficient method for ruling out drug presence in most samples, a positive result requires confirmation for legal admissibility. Any samples testing presumptively positive in Screening must undergo secondary confirmation.

If a sample shows a presumptive positive during Screening, additional hair is extracted from the original specimen and readied for Extraction. Here, drugs are isolated from hair at much lower concentrations than other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive result from screening is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation when necessary. Every step from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
