Hair is straightforward to collect, somewhat challenging to tamper with, and convenient to transport.

A 1.5-inch hair sample, consisting of roughly 200 strands (similar in size to a #2 pencil) closest to the scalp, yields 100mg of hair, which is optimal for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg specimen is advised. We suggest weighing the sample using a jeweler's scale. If scalp hair is unavailable, a comparable quantity of body hair can be gathered. Scalp hair pertains solely to head hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample within a lab's system, such as ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry that's missing from an electronic chain of custody system.

Screening involves an initial brief check for substances of abuse. While Screening is a cost-efficient method to dismiss drug use in the majority of samples, any positive screen must be verified for court admissibility. Samples that are presumptively positive in Screening require further confirmation.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo washing before confirmation if necessary. The overall laboratory procedure from Accessioning to Confirmation is evaluated according to the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
