What it is, How it works

Hair acts as a historical record of alcohol and drugs by embedding biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can reveal up to roughly a 3-month detection period for alcohol and other substances. Collecting hair is straightforward, moderately resistant to tampering, and convenient to transport.

A sample size of 1.5 inches, comprising about 200 hair strands (approximately the diameter of a #2 pencil) collected nearest the scalp, amounts to 100mg of hair, the preferred sample size for analysis and confirmation. For EtG, additional testing, and/or tests exceeding 10 panels, 150mg of hair is advisable. It is suggested to measure the sample using a jeweler’s scale. If scalp hair cannot be obtained, a comparable quantity of body hair may be used. When mentioning head hair, it specifically refers to scalp hair, whereas body hair includes other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial handling of a sample within a laboratory's system. This process entails verifying the proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry not provided through an electronic chain of custody system.

Screening constitutes an initial rapid assessment for drugs of abuse. Although Screening is an economical approach to exclude drug presence in the majority of samples, a positive result must be confirmed for legal admissibility. Samples with a presumptively positive Screening outcome necessitate further confirmation.

Should a sample appear presumptively positive during the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. This phase involves retrieving drugs from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening one of the most challenging methodologies.

Positive screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples are cleansed prior to confirmation if required. Every step of the laboratory process, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
