What it is, How it works

When obtained near the scalp, hair can reveal drug and alcohol use for a span of nearly three months. Hair collection is straightforward, relatively tamper-resistant, and convenient to transport.

A 1.5-inch section of roughly 200 hair strands (approximately matching the girth of a #2 pencil) closest to the scalp yields 100mg of hair, which is optimal for both screening and verification. For EtG, additional tests, and/or those exceeding 10 panels, a 150mg sample is ideal. We suggest using a jeweler’s scale for precise measurement. If scalp hair is not accessible, a comparable quantity of body hair may be gathered. When referring to head hair, we mean scalp hair only. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

Four primary phases are involved in a laboratory's drug test result processing: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s workflow. This process confirms the sample’s proper sealing and transport, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data input absent from an electronic custody chain.

Screening performs an initial rapid assessment for substance abuse. Although Screening is an economical method to dismiss drug presence in most samples, confirmatory tests are necessary for court admissibility. Any samples testing presumptively positive in Screening necessitate a further verification step.

In instances where the Screening stage indicates a presumptive positive, an additional hair portion is extracted from the original sample and readied for Extraction. During this procedure, substances are derived from hair at concentrations significantly lower than those in alternative methods (e.g., urine or oral fluid), which accounts for the complexity of hair drug screening.

Confirmation of any positive indications from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positives undergo washing as needed. The comprehensive lab process, from Accessioning to Confirmation, is assessed under CAP (College of American Pathologists) Hair specification and adheres to ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
