A 1.5-inch segment comprising approximately 200 strands of hair (around the thickness of a #2 pencil) nearest to the scalp will yield 100mg, which is the optimal amount for testing and verification. For EtG, supplemental tests, and/or analyses exceeding 10 panels, 150mg of the specimen is suggested. We suggest using a jeweler’s scale for measuring the specimen. If scalp hair cannot be obtained, an equivalent quantity of body hair may be used. When we mention head hair, we are specifically referring to scalp hair. Body hair denotes all other types of hair (such as facial, axillary, etc.).

Process Overview

The key stages in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample into a lab’s system. This ensures that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any further data entry not provided by an electronic custody chain system.

Screening entails an initial rapid analysis for substance misuse. Although Screening is a cost-efficient method to exclude drug usage in most samples, a positive result requires validation to be court-admissible. Samples that show presumptive positive results in Screening necessitate a secondary validation.

If a sample registers as presumptively positive in the Screening phase, more hair is retrieved from the core specimen and prepped for Extraction. During this phase, drugs are taken from hair at significantly lower concentrations than in alternative techniques (e.g., urine or oral fluid), thus making hair drug screening the most complex technique to execute.

Confirmation of any positive Screening result is accomplished via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The complete lab process from Accessioning to Confirmation undergoes review per the CAP (College of American Pathologists) Hair guidelines and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
