For EtG, additional tests, and/or tests beyond 10 panels, a specimen weight of 150mg is advised. Using a jeweler’s scale for weighing the sample is recommended. If scalp hair is not available, a similar quantity of body hair can be used. Reference to head hair implies scalp hair specifically, while body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

There are four key steps in laboratory processing of a drug testing result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of a sample's introduction into a laboratory system. This step involves confirming that the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry missing from the electronic chain of custody system.

Screening offers a preliminary quick evaluation for drug abuse. While Screening is a budget-friendly method to negate drug use in most samples, any positive screen must be confirmed to be valid for court use. Samples with a presumptive positive in Screening must undergo further confirmation.

Should a sample test presumptively positive during the Screening phase, additional hair is extracted from the initial sample and set up for Extraction. During this phase, drugs are isolated from hair in concentrations far lower than in other methods (e.g., urine or oral fluid), which makes hair drug testing the most complex method to execute.

Confirmation of any positive screening findings is performed through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are rinsed as necessary before confirmation. The entire process in the laboratory, from Accessioning to Confirmation, is examined under the CAP (College of American Pathologists) Hair designation and adheres to the ISO / IEC 17025 standards.
