What it is, How it works

When collected near the scalp, hair can offer a detection period for alcohol and drugs that extends to roughly three months. It's easy to gather, somewhat resistant to tampering, and simple to mail.

A 1.5-inch sample amounting to approximately 200 strands (similar in size to a #2 pencil) from near the scalp yields 100mg of hair, the optimal sample for both screening and confirmation. For EtG, additional tests, and/or for tests exceeding 10 panels, 150mg is advised. We suggest weighing the sample on a jeweler's scale. If head hair is not available, an equivalent quantity of body hair can be used. When mentioning head hair, we mean scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four critical steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the laboratory's system. This step includes confirming proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry not available through an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening is a cost-effective method to eliminate drug use in most samples, any positive screen must be verified for court admissibility. Samples that show a presumptive positive in Screening require further confirmation.

If a sample indicates a presumptive positive during Screening, additional hair is extracted from the collected specimen and prepared for Extraction. Here, drug extraction from hair occurs at much lower concentrations than other methods (such as urine or oral fluid), which makes hair testing the most challenging methodology to conduct.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive are washed before confirmation if necessary. The entire lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
