For EtG, add-ons, and tests exceeding 10 panels, 150mg of hair is advised. It's suggested to weigh the hair on a jeweler’s scale. If scalp hair is lacking, comparable body hair can be gathered. "Head hair" pertains solely to scalp hair, while "body hair" includes other types (like facial or axillary).

Process Overview

The primary stages in lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating a sample into the lab system. This phase ensures the sample is sealed and transported correctly, assigns it a random LAN (Laboratory Accessioning Number), and completes additional data entry absent from the electronic chain of custody.

Screening is a preliminary examination for drug presence. Despite its cost-effectiveness for most samples, positive results from Screening need confirmation to be valid in courtroom settings. All samples preliminarily positive must undergo secondary verification.

For presumed positives in Screening, additional hair is retrieved from the original sample for Extraction. Here, drug concentrations are extracted from hair at significantly lower levels compared to other methods (such as urine or oral swabs), which adds complexity to hair drug analysis.

Confirmation of positive screens occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are cleansed before confirmation if necessary. The complete lab process is vetted from Accessioning to Confirmation under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditations.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
