For EtG, additional tests, and/or panels exceeding 10, 150mg of hair is advised. It's advisable to weigh the specimen using a jeweler’s scale. If scalp hair is absent, an identical amount of body hair might be utilized. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary phases in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a specimen into the laboratory's system. This step involves confirming the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and inputting any supplementary data not already provided by an electronic chain of custody system.

Screening is a preliminary examination for drugs of abuse. While cost-effective, Screening only effectively eliminates drug use in most samples; a positive screen requires verification for court admissibility. Samples testing presumptively positive in Screening necessitate a follow-up Confirmation.

Upon a presumptive positive at the Screening phase, extra hair is extracted from the initial sample and processed for Extraction. Here, drug extraction from hair occurs at a concentration significantly lower than in other methods (e.g., urine or oral fluid), thus rendering hair drug testing exceptionally challenging.

Confirmation of positive screening outcomes is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptively positive specimens are washed before confirmation as necessary. Every step in the lab process from Accessioning to Confirmation is scrutinized under CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
