What it is, How it works

Hair testing now stands as an effective mechanism for identifying drug and alcohol consumption. Hair showcases an extended history of alcohol and drugs by embedding biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can offer up to nearly a 3-month detection window for alcohol and other substances. Hair is straightforward to collect, somewhat hard to tamper with, and convenient to ship.

A 1.5-inch portion comprising about 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample size for screening and confirmation tests. For EtG, additional panels, and/or tests over 10 panels, 150mg of the sample is advisable. Utilizing a jeweler’s scale to determine the specimen's weight is recommended. If scalp hair is missing, an equivalent amount of body hair can be gathered. With regards to head hair, we specifically mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The process for analyzing a drug test result in the laboratory encompasses Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a lab's system. This includes confirmation that the sample was securely sealed and correctly shipped, assignment of a random LAN (Laboratory Accessioning Number), and completing any extra data entry not available through an electronic chain of custody system.

Screening entails an initial swift evaluation for drugs of abuse. While Screening serves as an economical approach to exclude drug use in most samples, a positive result necessitates confirmation for legal admissibility. Samples that yield a presumptive positive in Screening need additional confirmation.

If a sample tests positive in the Screening stage, further hair is taken from the original specimen to prepare for Extraction. In this stage, drugs are extracted from hair at considerably lower concentrations than in other methods (e.g., urine or oral fluid), which accounts for the complexity of hair drug screening methodology.

Confirmation of any positive result from the screening phase occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo washing before confirmation, as required. The entire lab process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair classification and accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
