What it is, How it works

A 1.5-inch hair sample consisting of around 200 strands (comparable to a #2 pencil) nearest the scalp will yield 100mg of hair, the preferred quantity for screening and verification. For EtG, additional tests, and/or panels over 10, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the hair. If the scalp hair is not present, a similar amount of body hair can be collected. Head hair refers specifically to scalp hair, while body hair refers to hair on the body (e.g., facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the step where a sample is initially logged into the laboratory's system. This involves ensuring the sample was properly sealed and delivered, assigning a unique LAN (Laboratory Accessioning Number), and handling any extra data entry not covered by an electronic chain of custody.

Screening is a preliminary examination for substances of abuse. While Screening provides a cost-efficient way to eliminate drug use in most samples, positive results must be validated to be courtroom-acceptable. Any samples preliminarily positive in Screening require a follow-up confirmation.

When a sample shows a probable positive during the Screening stage, additional hair is extracted from the original sample and prepped for Extraction. At this stage, drugs are extracted from hair at much lower levels than other methods (e.g., urine or oral fluid), making hair drug testing the most challenging procedure to execute.

Confirmation of any positive screen is done through GC/MS, GC/MS/MS, or LC/MS/MS. All potential positive samples are washed prior to confirmation if necessary. From Accessioning to Confirmation, the complete laboratory process is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
