A hair sample measuring 1.5 inches, consisting of around 200 strands (equivalent to the thickness of a #2 pencil) closest to the scalp will provide 100mg, which is the optimal sample size for screening and validation. For EtG, additional tests, and/or tests exceeding 10 panels, a specimen mass of 150mg is preferred. We advise using a jeweler’s scale for weighing the sample. If scalp hair is not available, a comparable amount of body hair can be collected. When mentioning head hair, it specifically refers to scalp hair, while body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The four primary stages in processing a drug test result in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial handling of a sample within a lab’s workflow. This step verifies that the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data that isn’t integrated by an electronic chain of custody system.

Verification of any positive screening result occurs through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, any presumptive positive samples undergo washing as needed. The complete lab process from Accessioning to Confirmation is systematically reviewed under both the CAP (College of American Pathologists) Hair designation and the compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
