What it is, How it works

Hair testing is recognized as an effective instrument for identifying drug and alcohol consumption. Hair can capture a long-term record of drugs and alcohol by embedding biomarkers within the hair strand as it grows. When obtained near the scalp, hair enables detection of alcohol and drugs for up to nearly 90 days. Hair is straightforward to collect, hard to tamper with, and convenient to transport.

A 1.5-inch section of around 200 hair strands (similar in size to a #2 pencil) cut closest to the scalp results in 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, add-ons, or tests beyond 10 panels, 150mg of the hair sample is ideal. We advise using a jeweler’s scale to weigh the sample. If scalp hair isn't accessible, an equal quantity of body hair can be used. By head hair, we mean only scalp hair, whereas body hair represents other types (such as facial, or axillary).

Process Overview

There are four key stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step, where samples are initially processed into a lab's system. Tasks involve ensuring the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not managed by an electronic chain of custody system.

Screening entails a preliminary inspection for the presence of drugs of abuse. Although Screening is a cost-efficient means to exclude drug usage in most samples, a positive result requires confirmation to be valid in a legal setting. Presumptively positive samples in Screening necessitate further confirmation.

If a sample appears positive during the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. At this stage, drugs are separated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

GC/MS, GC/MS/MS, or LC/MS/MS methods are used to confirm any positive Screening results. All samples considered presumptively positive are cleansed before confirmation when necessary. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
