Collecting a 1.5-inch segment of roughly 200 hair strands (similar in size to a #2 pencil) near the scalp provides 100mg, which is ideal for both initial screening and confirmation. For EtG, additional substances, or tests exceeding 10-panel, 150mg is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair is unavailable, equivalent body hair may be gathered. When discussing head hair, we're specifically referring to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result entails four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the sample’s initial integration into the laboratory's system, ensuring it was correctly sealed and sent, generating a unique LAN (Laboratory Accessioning Number), and filling in any supplementary data entry missing from an electronic chain of custody system.

Screening serves as an initial assessment for substances of abuse. While cost-effective for eliminating drug use in most samples, a positive screen requires confirmation to be legally valid. Samples that yield presumptively positive results during Screening need a subsequent verification.

Should a sample appear positive in the Screening phase, additional hair is drawn from the original sample for Extraction. In this phase, drugs are isolated from hair at lower concentrations compared to alternatives (e.g., urine, oral fluid), making hair drug screening the most intricate to execute.

Verification of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are soaked before confirmation as necessary. The entire lab process, from Accessioning through Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
