For EtG, add-ons, and/or tests exceeding 10 panels, acquiring 150mg of the sample is advisable. It is suggested to measure the sample on a jeweler’s scale. In the absence of scalp hair, an equivalent mass of body hair may be used. When discussing head hair, we mean specifically scalp hair. Body hair includes all other hair varieties (facial, axillary, etc.).

Process Overview

The primary stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a lab’s system. This process ensures the sample was sealed and transported correctly, assigns a random LAN (Laboratory Accessioning Number), and finalizes any remaining data entry absent from an electronic chain of custody system.

Screening is a preliminary assessment for drugs of abuse. While Screening is an economical method to exclude drug use in most samples, a positive screen necessitates confirmation to be valid in court. Samples presumptively positive in Screening must undergo secondary confirmation.

Should a sample be presumptive positive during Screening, additional hair is gathered from the original sample and readied for Extraction. During Extraction, drugs are isolated from hair at a considerably reduced concentration than other methods (e.g., urine or oral fluid), making hair drug screening a challenging methodology to execute.

Confirmation of any presumptive positive finding is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as necessary. The complete lab procedure, from Accessioning to Confirmation, undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
