If scalp hair is unavailable, a comparable amount of body hair is suitable. When specifying head hair, it pertains only to scalp hair. Body hair encompasses all other hair types (such as facial or underarm).

Process Overview

The process in the lab to generate a drug test outcome involves four major stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the first step of a sample’s journey through a lab’s system. This involves ensuring the sample was sealed and dispatched correctly, generating a random LAN (Laboratory Accessioning Number), and completing any extra data input not covered by an electronic chain of custody process.

Screening involves a preliminary quick assessment for substances of abuse. Despite being a cost-efficient approach to eliminate potential drug use in most samples, a positive screening result must be validated for court acceptance. Any samples showing presumptive positives from Screening are required to undergo further verification.

In cases where a sample tests presumptively positive at the Screening phase, additional hair is extracted from the original sample and readied for Extraction. In Extraction, drugs are extracted from hair at a significantly lower concentration than is typical with other methods (like urine or oral fluid), highlighting hair drug screening as the most complex method to execute.

Any positive screening outcomes are confirmed via technologies such as GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washes before confirmation if needed. The complete lab process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
