If scalp hair isn't available, an equivalent quantity of body hair can be used. In the context of head hair, we refer exclusively to scalp hair. Body hair includes other types such as facial or axillary hair.

Process Overview

The lab processing of a drug test result includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the provisional processing of a sample within a lab's system. This procedure ensures the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry not covered by an electronic chain of custody system.

Screening is an initial rapid review for drugs of abuse. Although Screening is a cost-effective approach for eliminating drug use in many samples, any positive findings must be validated to stand in court. Thus, all samples that test presumptively positive in Screening require further confirmation.

If the Screening phase suggests a positive result, additional hair is extracted from the original specimen and prepared for Extraction. During this stage, drugs are sourced from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

All positive screenings are verified via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, presumptive positive samples undergo washing as required. From Accessioning to Confirmation, the laboratory procedures are evaluated according to CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
