What it is, How it works

Hair testing is recognized as a substantial method for detecting drug and alcohol consumption. Hair provides a prolonged history of alcohol and other substances by trapping biomarkers within the fibers of the hair strand as it grows. When sampled near the scalp, hair can reveal up to a 3-month period for detecting alcohol and other substances. Hair collection is straightforward, somewhat challenging to tamper with, and convenient to transport.

A 1.5-inch section of around 200 hair strands, approximately the width of a #2 pencil, nearest to the scalp, results in 100mg of hair, which is an ideal amount for analysis and verification. For EtG and tests exceeding 10 panels, 150mg of the specimen is suggested. We advise using a jeweler’s scale for weighing the sample. In instances where scalp hair isn't available, an equivalent quantity of body hair can be collected. Head hair specifically refers to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial preparation of a sample within a laboratory’s framework. This stage includes checking that the sample was appropriately sealed and transported, generating a random LAN (Laboratory Accessioning Number), and finalizing any data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of concern. Although Screening is a cost-efficient approach to identify drug usage in many samples, a positive result must be verified to be acceptable in legal settings. Samples that test positive in Screening require a subsequent confirmation stage.

If a sample tests positive in the Screening phase, additional hair is extracted from the initial sample for the Extraction process. Here, drugs are isolated from hair at a much lower concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation is carried out for any positive screening via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test positive are washed before confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Difficult to manipulate: It is challenging to manipulate a hair drug test, thus enhancing accuracy.

  • Incapable of detecting recent intake: Drugs become detectable in hair roughly 5-7 days post-consumption.

Note: Although commonly called "hair follicle tests", the examination analyzes the hair itself, not the follicle beneath the scalp.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
