What it is, How it works

When taken close to the scalp, hair can offer a detection period of up to about three months for alcohol and drugs. Hair is straightforward to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch hair specimen, approximately 200 strands (around the diameter of a #2 pencil) nearest to the scalp, yields 100mg of hair, the preferred volume for testing. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the hair sample is advised. Utilizing a jeweler’s scale for weighing the sample is recommended. Should scalp hair be unavailable, equivalent body hair can be used. When we mention head hair, we mean scalp hair only. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four critical steps in lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial processing of the specimen into a laboratory’s system. This involves confirming the proper sealing and shipping of the sample, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any further data entry absent from an electronic chain of custody system.

Screening involves a preliminary rapid test for drugs. While Screening is an efficient way to exclude drug use from most samples, a positive screen must be validated to be acceptable in court. Any samples initially positive in Screening necessitate a further confirmation step.

If Screening indicates a presumptive positive, more hair is drawn from the original sample for Extraction. Here, drugs are extracted from hair at a much lower concentration compared to other methods (e.g., urine or saliva), making hair drug tests one of the most challenging methodologies to conduct.

Any positive Screening result is validated via techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as necessary. The lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
