Hair analysis has emerged as an effective method for identifying drug and alcohol consumption. Hair provides a long-term historical record of alcohol and other drug use by embedding biomarkers in the strands of hair as it grows. When taken close to the scalp, hair can offer an approximate 3-month detection timeline for alcohol and drugs. Hair collection is straightforward, somewhat resistant to tampering, and easy to transport.
A 1.5-inch sample consisting of about 200 hair strands (comparable in size to a #2 pencil) proximate to the scalp will yield 100mg of hair, considered the optimal sample for testing and verification. For EtG, additional tests, and/or those exceeding 10 panels, a specimen of 150mg is advised. We suggest using a jeweler’s scale to gauge the specimen’s weight. Should scalp hair be absent, a comparable quantity of body hair can be used. When discussing head hair, we specifically refer to scalp hair, whereas body hair includes all other types (facial, axillary, etc.).
Process Overview
The four critical stages involved in lab processing of drug test results are Accessioning, Screening, Extraction, and Confirmation.
Accessioning entails the initial integration of a sample into the lab’s system. This process includes ensuring the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry that an electronic chain of custody doesn’t cover.
Screening involves an initial rapid assessment for illegal substances. While Screening is a cost-effective approach to dismiss drug use in most samples, a positive screen must be confirmed for legal admissibility. Any samples with presumptive positives during Screening must undergo a secondary confirmation.
In cases where a sample presents as presumptively positive during Screening, additional hair is extracted from the original specimen and readied for Extraction. Here, drugs are isolated from hair at a substantially lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug analysis is the most challenging to conduct.
Confirmation of any positive results from screening is carried out through GC/MS, GC/MS/MS or LC/MS/MS. All samples deemed presumptively positive are cleansed before confirmation if necessary. From Accessioning to Confirmation, the complete laboratory procedure adheres to CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 accreditation standards.
