A 1.5-inch sample comprising approximately 200 strands of hair (roughly the size of a #2 pencil) nearest to the scalp provides 100mg of hair, the optimal sample for analysis and validation. For EtG, supplemental, and/or evaluations exceeding 10 panels, 150mg of the specimen is advisable. We advocate for measuring the sample on a jeweler’s scale. If scalp hair is not obtainable, an equivalent quantity of body hair might be used. When speaking of head hair, we specifically refer to scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory evaluation of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory’s framework. This entails confirming that the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry not supplied by an electronic chain of custody system.

Screening signifies an initial rapid assessment for substance abuse. While Screening is an economical way to negate drug usage on the majority of samples, a positive screen must be validated to be admissible in court. Any samples that are presumptively positive in Screening necessitate a subsequent confirmation.

If a sample is presumptively positive during the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (ex. urine or oral fluid), making hair drug screening the most challenging method to perform.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The entire laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
