Collection is straightforward, tampering is challenging, and shipping is convenient.

A 1.5-inch segment of roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is optimal for screening and confirmation. For EtG, add-ons, and tests beyond 10 panels, 150mg of the sample is suggested. Using a jeweler’s scale is advised for specimen weighing. If scalp hair isn't accessible, a comparable amount of body hair can be used. References to head hair imply scalp hair, while body hair includes all other types, such as facial and axillary hair.

Process Overview

The laboratory processing of a drug test result consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into the lab's system, ensuring proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data entry not captured by an electronic chain of custody.

Screening involves an initial swift assessment for drug abuse. It offers a cost-efficient means to dismiss drug use for most samples; however, positive screens must be confirmed for legal acceptance. Samples presumptively positive in Screening require additional confirmation.

Confirmation of any positive screening is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation, as necessary. The entire process from Accessioning to Confirmation is reviewed under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
