What it is, How it works

The use of hair testing has emerged as an influential method for identifying the consumption of drugs and alcohol. Hair strands capture biomarkers within their fibers, offering a prolonged record of alcohol and drug exposure. By taking a sample near the scalp, hair can provide a detection timeframe of up to roughly three months for drugs and alcohol. Collecting hair is straightforward, somewhat resistant to tampering, and convenient to transport.

A segment of 1.5 inches encompassing about 200 hair strands (comparable to the thickness of a #2 pencil) next to the scalp will yield 100mg of hair, which is the ideal amount for screening and confirmation. When conducting EtG, add-ons, or tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler's scale to weigh the specimen is recommended. If scalp hair isn't available, an equivalent quantity of body hair can be collected. When mentioning head hair, we refer solely to scalp hair. Body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial intake of a sample into a laboratory's system. This process ensures the sample is properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and finalizes any data entry absent from an electronic chain of custody system.

Screening is a preliminary assessment for drugs of abuse. While it serves as an economical method to exclude drug usage for most samples, a positive result from Screening must be validated to be court-admissible. Any samples presenting presumed positivity in Screening require follow-up confirmation.

In circumstances where a sample shows presumed positivity in the Screening phase, additional hair is extracted from the original sample and made ready for Extraction. During this phase, the extraction of drugs from hair occurs at significantly lower concentrations than alternative methods (such as urine or oral fluid), thus making hair drug screening particularly challenging.

Positive screening results undergo Confirmation by means of GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positives are washed as necessary. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

Advantages of hair drug testing:

  • Extended detection period: Hair drug tests are capable of detecting drug use up to 90 days, unlike urine tests which offer a shorter detection timeframe.
  • Challenging to bypass: The high difficulty of tampering with hair drug tests lends greater accuracy to the results.
  • Yields historical insight: Hair tests can reveal drug use patterns over time, instead of merely recent usage.

Limitations:

  • Unable to discern recent usage: It takes approximately 5-7 days for drugs to become detectable in hair.
  • Expense: Hair drug tests tend to be costlier than other drug testing methodologies.
  • Result fluctuations: Variables such as hair color and individual hair growth differences can impact drug metabolite concentrations in hair.

Note: Although often termed "hair follicle tests", the test itself evaluates the hair strand, not the follicle beneath the scalp.
