A specimen of 1.5 inches comprising around 200 hair strands (about the thickness of a #2 pencil) close to the scalp yields 100mg of hair, the preferred sample size for examination and verification. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of sample is advisable. Using a jeweler's scale to weigh the sample is suggested. If scalp hair isn't available, a corresponding amount of body hair can be collected. References to head hair are specific to scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory procedures for processing a drug test include four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step of integrating a sample into a lab's system. It covers verifying the sample's proper sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not captured by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. Although Screening offers a cost-efficient means to dismiss drug use in most samples, a confirmed screen is necessary for legal admissibility. Any samples flagged as presumptively positive in Screening necessitate a subsequent confirmation.

When a sample tests presumptively positive in Screening, an additional portion is extracted from the original sample and processed for Extraction. In this phase, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), reflecting the complexity of hair drug screening.

Confirmation of any positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples noted as presumptive positives undergo required washing before confirmation. The complete laboratory procedure, from Accessioning to Confirmation, is reviewed per both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
