In scenarios where scalp hair is unavailable, a comparable volume of body hair can be gathered. When discussing head hair, we are specifically indicating scalp hair alone. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The process for analyzing drug test outcomes in a lab includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a specimen into a lab’s system. This process involves verifying that the specimen was appropriately sealed and transported, generating an arbitrary LAN (Laboratory Accessioning Number), and finalizing any supplementary data input absent in an electronic chain of custody system.

Screening consists of a preliminary quick assessment for substance abuse. While Screening is a budget-friendly approach to eliminate drug usage in most specimens, a positive outcome in Screening requires validation to be permissible in a legal setting. Any specimens that register a presumptive positive in Screening necessitate a follow-up confirmation.

Should a specimen indicate a presumptive positive in the Screening stage, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, substances are extracted from hair at a notably lower concentration compared to alternative methodologies (e.g., urine or oral fluid), underscoring why hair drug screening is the most challenging to execute.

Verification of any positive Screening result is undertaken via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed prior to confirmation if required. The comprehensive lab procedure from Accessioning to Confirmation is subjected to review under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
