What it is, How it works

Hair analysis is recognized as an effective method for detecting substance and alcohol consumption. Hair offers a prolonged record of substance intake by embedding markers within the strands as they grow. When taken near the scalp, hair can reveal drug and alcohol usage within an approximately three-month period. Hair collection is straightforward, less prone to tampering, and convenient for transport.

A 1.5-inch sample comprising roughly 200 hair strands (akin to the thickness of a #2 pencil) nearest to the scalp will yield 100mg, which is perfect for initial testing and confirmation. For EtG, extended panels, and/or tests exceeding 10 panels, a 150mg sample is advisable. Utilize a jeweler’s scale for precise measurement. If head hair is absent, an equivalent mass of body hair may be utilized. Note that head hair pertains solely to scalp hair. Body hair encompasses all other types (such as facial, underarm, etc.).

Process Overview

The key phases in laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves incorporating the sample into the lab system. This step confirms the sample’s proper sealing and shipping, allocates a random LAN (Laboratory Accessioning Number), and completes any outstanding data entry not covered by an electronic custody system.

Screening performs an initial rapid assessment for substance abuse. Although Screening is an economical approach to exclude drug use in most samples, court-acceptable results require positive screens to undergo confirmation. All samples showing indicative positive results in Screening need subsequent confirmation.

Should a sample yield a presumptive positive during Screening, additional hair is taken from the primary sample for Extraction preparation. Drugs are isolated from hair at lower concentrations compared to other methods (e.g., urine or saliva), making hair screening particularly complex.

Positive results then undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive samples are cleansed prior to confirmation if necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized under both CAP Hair designation and ISO / IEC 17025 accreditation.

Advantages of hair drug testing:

  • Extensive detection period: Hair testing can trace drug use for up to 90 days, contrasting with the shorter timeframe of urine tests.
  • Hard to evade: Evasion of a hair test is challenging, ensuring greater result accuracy.
  • Reveals usage patterns: It demonstrates drug use trends over time, not just immediate use.

Limitations:

  • Unable to detect recent usage: Drugs need about 5-7 days to appear in hair samples.
  • Expense: Hair tests are typically costlier than other testing types.
  • Result variability: Hair color and personal growth factors can influence drug metabolite concentration in hair.

Note: Often labeled as "hair follicle tests", these tests actually analyze the strand itself rather than the follicle beneath the scalp.
