What it is, How it works

Hair captures a prolonged record of alcohol and drug use by embedding biomarkers within the fibers of the maturing hair strand. Hair gathered near the scalp can reveal up to nearly a 3-month timeframe for detecting alcohol and drugs. Hair is straightforward to collect, relatively resistant to tampering, and easy to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for screening and validation. For EtG, additional tests, and/or tests exceeding 10 panels, it is recommended to obtain 150mg of the sample. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, a similar quantity of body hair may be collected. When referring to head hair, we specifically mean scalp hair. Body hair refers to all other hair types (facial, axillary, etc.).

Process Overview

The central stages in the lab processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This encompasses confirming the sample was securely sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and inputting any additional data not covered by an electronic chain of custody system.

Screening comprises an initial cursory inspection for drugs of abuse. Although Screening is a cost-efficient method for excluding drug use in most samples, a positive screen must be verified to be permissible in court. Any samples that test presumptively positive in Screening need further confirmation.

If a sample tests presumptively positive during Screening, additional hair is taken from the original specimen and readied for Extraction. At this phase, drugs are separated from the hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to perform.

Verification of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is reviewed per the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
