What it is, How it works

Hair testing has emerged as an effective tool for identifying drug and alcohol usage. Hair retains a long-term record of alcohol and drug consumption by encapsulating biomarkers within the strands as they grow. When sampled near the scalp, hair can offer a detection window of up to nearly 3 months for drugs and alcohol. Hair is straightforward to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch section of about 200 hair strands (approximately the thickness of a #2 pencil) near the scalp yields 100mg of hair, the preferred sample amount for screening and confirmation. In cases of EtG, add-ons, or tests exceeding 10 panels, a 150mg sample is advised. We suggest measuring the sample on a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may be utilized. References to head hair specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves integrating the sample into the lab’s system, verifying that it was securely sealed and correctly shipped, assigning it a LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody.

Screening serves as an initial, economical assessment for drugs of abuse. While efficient for eliminating drug use suspicions on most samples, a positive screen must be confirmed for legal admissibility. Samples that show presumptive positive results in Screening necessitate further confirmation.

If a sample appears positive in the Screening phase, additional hair is used from the initial sample and prepared for Extraction. This phase extracts drugs from hair in notably lower concentrations compared to methods like urine or oral fluid, highlighting the complexity of hair drug screening.

Confirmation of any positive screenings is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if needed. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
