Its main advantage is a long detection window of up to 90 days, through a supervised collection method, making it hard to falsify. However, within the last 5 days, urine tests remain most precise.

What it is, How it works

Hair testing is now regarded as an effective method for identifying drug and alcohol consumption. Hair offers an extended historical record of alcohol and drug use by capturing biomarkers within the fibers of the hair as it grows. When collected near the scalp, hair can demonstrate a detection period up to roughly three months for substances such as alcohol and drugs. The collection of hair is straightforward, challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches and consisting of approximately 200 hair strands (about comparable to a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal quantity for analysis and verification. For EtG, add-ons, and/or evaluations exceeding 10 panels, 150mg of the sample is advocated. We suggest measuring the specimen using a jeweler’s scale. If scalp hair is not available, an equivalent portion of body hair can be gathered. By head hair, we mean solely scalp hair. Body hair encompasses all other types (e.g., facial, axillary, etc.).

Process Overview

The four primary phases in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a lab's system. This entails confirming that the sample was appropriately sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and inputting any supplementary data not handled by an electronic chain of custody system.

Screening includes an introductory rapid assessment for substances of abuse. While Screening is a budget-friendly approach to dismiss drug usage in most samples, a positive screen must be verified for court admissibility. Any samples that presumptively test positive in Screening do require a subsequent confirmation.

When a sample is presumptively positive during the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. At this stage, drugs are withdrawn from hair at substantially lower concentrations compared to other procedures (e.g., urine or oral fluid), making hair drug screening the most intricate process to perform.

Verification of any positive screening outcome is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing before confirmation if required. The entire lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
