What it is, How it works

Hair captures long-term evidence of alcohol and other substances by embedding biomarkers within the hair strand's fibers as it grows. When cut near the scalp, hair can offer a detection period of up to roughly three months for alcohol and other substances. Collecting hair is straightforward, relatively tamper-proof, and easily transported.

A 1.5-inch section consisting of about 200 hair strands (similar in size to a #2 pencil) nearest to the scalp produces 100mg of hair, which is the optimal sample size for both screening and verification. For EtG, supplementary tests, and/or tests beyond 10 panels, a 150mg sample is advisable. We suggest measuring the specimen on a jeweler's scale. If scalp hair is not available, an equivalent volume of body hair may be used. "Head hair" refers solely to scalp hair. "Body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

The key stages in the lab process for a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step where a sample is integrated into a lab's system. It involves confirming the sample was correctly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any missing data not provided by an electronic chain of custody procedure.

Screening serves as an initial, efficient check for drugs of abuse. Although Screening offers an economical option to exclude drug use in most cases, a positive screen must be validated to be legally acceptable. Any samples that show initial positive results in Screening require further confirmation.

Should a sample test positive at the Screening stage, additional hair is extracted from the original sample and prepared for Extraction. At this stage, drugs are isolated from hair at significantly reduced concentration levels compared to other methods (e.g., urine or oral fluid), rendering hair drug screening as the most challenging approach to execute.

Confirmation for any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Any preliminary positive samples are cleansed before confirmation if necessary. The full lab process from Accessioning to Confirmation is subjected to review under both CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.
