For EtG, supplements, and/or tests exceeding 10 panels, a 150mg sample is advisable. We suggest using a jeweler’s scale to measure the specimen. If scalp hair isn't available, a comparable amount of body hair can be used. By head hair, we specifically mean scalp hair. Body hair encompasses all other hair forms (like facial, axillary, etc.).

Process Overview

The primary phases in laboratory drug test result processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample into a lab's system, which includes confirming the proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing missing data not provided by an electronic chain of custody system.

Screening involves a preliminary rapid analysis for drugs of abuse. While screening is an economical method to rule out drug use for most samples, a positive screening result demands confirmation to be valid for court use. Any samples showing presumptive positive results in screening necessitate further confirmation.

If a sample tests presumptive positive during Screening, additional hair is extracted from the initial sample for the Extraction phase. This stage involves extracting drugs from hair at a much lower concentration than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of positive screening results uses GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as needed. The comprehensive laboratory workflow from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair criteria and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
