A 1.5-inch hair sample, roughly 200 strands (comparable to a #2 pencil thickness), provides 100mg of hair, which is the optimal sample size for testing and verification. For EtG, additional tests, and/or panels beyond ten, a 150mg specimen is suggested. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair is unavailable, an equivalent amount of body hair can be collected. In this context, head hair refers solely to scalp hair, while body hair includes all other types (such as facial, axillary, etc.).

Process Overview

The key stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with the preliminary processing of a sample into the laboratory's system. It verifies the sample was correctly sealed and sent, assigns a LAN (Laboratory Accessioning Number) randomly, and concludes any extra data input not covered by an electronic chain of custody system.

Screening performs an initial rapid assessment for drug abuse. Although Screening is an economical method to eliminate drug use in most samples, a positive screen requires confirmation to be acceptable in court. Any samples with a presumptive positive result in Screening need a subsequent confirmation.

If Screening indicates a presumptive positive result, extra hair is extracted from the initial sample and set for Extraction. During this phase, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), highlighting hair drug screening's complexity.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed before confirmation if necessary. The entire lab process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.
