A sample of 1.5 inches, consisting of approximately 200 hair strands (similar to the thickness of a #2 pencil), collected closest to the scalp, will yield 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10 panels, 150mg of the sample is advised. We suggest weighing the sample with a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair can be used. Reference to head hair pertains strictly to scalp hair. Body hair includes various other types of hair (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial stage of integrating a sample into a laboratory's system. This step includes confirming the sample was sealed and transported correctly, generating a random LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening includes a preliminary, rapid evaluation for drugs of abuse. While Screening is an economical approach to eliminate drug use in most samples, a positive result from the screen must be verified to be valid in legal settings. Any samples with a presumptive positive result in Screening must undergo further confirmation.

For samples that return a presumptive positive result during the Screening phase, extra hair is taken from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation for any positive screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed prior to confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation undergoes review according to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
