A 1.5-inch hair sample, comprising around 200 strands (equivalent to the width of a #2 pencil) near the scalp, provides 100mg of hair, which is the optimal amount for both initial screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg is advisable. The sample should be weighed using a jeweler’s scale. If scalp hair isn't accessible, a similar quantity of body hair is acceptable. When mentioning head hair, we mean only scalp hair. Body hair refers to all other kinds of hair (facial, axillary, etc.).

Process Overview

The processing of drug test results in a lab involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is entered into the lab’s system. This step checks that the sample is sealed and shipped correctly, assigns a LAN (Laboratory Accessioning Number) randomly, and completes any extra data entry that an electronic chain of custody system doesn’t cover.

Screening serves as a preliminary evaluation for drug presence. Although cost-effective for excluding drug use in most samples, a positive preliminary result necessitates confirmation for legal admissibility. Samples with potential positive results in Screening need further verification.

If Screening suggests a positive result, more hair is extracted from the original sample for further analysis. Drugs are isolated from hair at much lower concentrations compared to other methods (like urine or oral fluid), which is why hair drug testing is among the most challenging techniques.

Confirmation of a positive test is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. Samples indicating presumptive positives undergo a cleansing process before confirmation, as needed.
