Hair follicle testing has increasingly become a favored method for drug examinations recently. Various employers, judicial systems, and substance abuse specialists prefer this over the traditional urine test. These tests are vital for organizations with strict drug policies, legal entities, and probation officers, thanks to their extended 90-day detection timeframe, direct observation during collection, and difficulty to tamper with, though urine tests excel in identifying substance use within a 5-day window.

What it is, How it works

Hair analysis is now recognized as an effective means to identify the consumption of drugs and alcohol. The hair captures a lasting record of alcohol and drug use by embedding biomarkers within the strands as they grow. When hair is sampled near the scalp, it can reveal alcohol and drug use over roughly a 3-month period. Hair collection is easy, tampering is challenging, and transportation is straightforward.

A 1.5-inch hair sample, comprising around 200 strands (equivalent to the width of a #2 pencil) near the scalp, provides 100mg of hair, which is the optimal amount for both initial screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg is advisable. The sample should be weighed using a jeweler’s scale. If scalp hair isn't accessible, a similar quantity of body hair is acceptable. When mentioning head hair, we mean only scalp hair. Body hair refers to all other kinds of hair (facial, axillary, etc.).

Process Overview

The processing of drug test results in a lab involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is entered into the lab’s system. This step checks that the sample is sealed and shipped correctly, assigns a LAN (Laboratory Accessioning Number) randomly, and completes any extra data entry that an electronic chain of custody system doesn’t cover.

Screening serves as a preliminary evaluation for drug presence. Although cost-effective for excluding drug use in most samples, a positive preliminary result necessitates confirmation for legal admissibility. Samples with potential positive results in Screening need further verification.

If Screening suggests a positive result, more hair is extracted from the original sample for further analysis. Drugs are isolated from hair at much lower concentrations compared to other methods (like urine or oral fluid), which is why hair drug testing is among the most challenging techniques.

Confirmation of a positive test is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. Samples indicating presumptive positives undergo a cleansing process before confirmation, as needed. The whole lab procedure, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation and accreditation standards ISO / IEC 17025.

Advantages of hair drug testing:

  • Extended detection timeline: Hair tests can identify drug use for up to 90 days, unlike urine tests, which detect use over a shorter period.
  • Challenging to falsify: Cheating a hair test is very difficult, ensuring result accuracy.
  • Records historical data: Offers an overview of drug use patterns over time, not just recent usage.

Limitations:

  • Inability to detect recent use: Drugs take around 5-7 days to become detectable in hair.
  • Higher cost: Hair testing typically incurs higher expenses compared to other drug tests.
  • Result variability: Factors such as hair color and individual hair growth differences can alter drug metabolite concentrations in hair.

Note: Although often called "hair follicle tests," the actual analysis is performed on the hair strand, not the follicle beneath the scalp.
