What it is, How it works

Hair examination has emerged as an influential means for detecting drug and alcohol consumption. Hair offers a prolonged record of alcohol and other substances by embedding biomarkers within the fibers of the growing hair strand. When collected close to the scalp, hair can indicate up to an approximate 3-month detection timeframe for alcohol and other substances. Hair collection is straightforward, rather resistant to tampering, and convenient to transport.

A 1.5-inch segment encompassing about 200 hair strands (comparable to the girth of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for analysis and confirmation. For EtG, additional screenings, and/or tests exceeding 10 panels, 150mg of the sample is advised. We recommend using a jeweler’s scale to weigh the sample. In cases where scalp hair is unavailable, a similar amount of body hair can be collected. The reference to head hair pertains solely to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four fundamental stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample within a laboratory's system. This entails confirming that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening entails an initial rapid check for substances of abuse. While Screening is a cost-efficient method to rule out substance use on the majority of samples, a positive result requires confirmation to be admissible in court. Any samples that are presumed positive in Screening necessitate a secondary confirmation.

If a sample is presumed positive at the Screening stage, additional hair is extracted from the initial specimen and prepared for Extraction. At this stage, substances are extracted from hair at a significantly lower concentration compared to other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples are cleansed before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair certification and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
