What it is, How it works

Hair retains a long-term record of alcohol and drugs by encapsulating biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can offer approximately a 3-month detection window for alcohol and drugs. The collection of hair is straightforward, moderately resistant to tampering, and simple to ship.

A 1.5-inch sample consisting of roughly 200 strands of hair (around the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal sample volume for screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, a 150mg specimen is advised. We suggest using a jeweler’s scale for weighing the specimen. If scalp hair is unavailable, an equal portion of body hair can be collected. "Head hair" refers specifically to scalp hair. "Body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

There are four primary stages in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of sample processing in a laboratory’s system. This involves checking that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. While Screening is a cost-effective method for ruling out drug usage across most samples, a positive screen must be confirmed to be legally valid. All samples that are presumptive positives in Screening necessitate a secondary confirmation.

Confirmation of any positive screening results is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
