What it is, How it works

A sample measuring 1.5 inches with approximately 200 hairs (around the thickness of a #2 pencil) near the scalp yields 100mg, considered optimal for both screening and verification. For EtG, add-ons, or tests exceeding 10 panels, 150mg of the sample is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair can be used. The term head hair refers exclusively to scalp hair, while body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The drug testing procedure involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial processing of a sample into the lab system. This step comprises verifying the proper sealing and shipment of the sample, assigning a random Laboratory Accessioning Number (LAN), and completing any additional data entry that isn't part of the electronic chain of custody system.

Screening entails a preliminary assessment for drugs of abuse. Although Screening efficiently eliminates the likelihood of drug use in many samples, a positive result necessitates confirmation to be legally admissible. Samples that yield a presumptive positive in Screening must undergo secondary confirmation.

When a sample returns a presumptive positive during Screening, more of the original sample is retrieved and readied for Extraction. At this stage, drugs are extracted from hair at much lower concentrations compared to other methods (such as urine or oral fluid), rendering hair drug screening a challenging practice.

Confirmation of a positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are washed before confirmation as required. The entire assessment process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and compliance with ISO/IEC 17025 standards.
