For EtG, additional tests, and/or more than 10 panels, 150mg of the specimen is advisable. It is suggested to use a jeweler’s scale for weighing the specimen. If scalp hair is not available, the same quantity of body hair can be collected. When referring to head hair, it should only mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result includes four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves processing the sample into the lab’s system initially. This step includes ensuring the sample was sealed and shipped correctly, providing a random LAN (Laboratory Accessioning Number), and filling out any additional data entry not included in an electronic chain of custody system.

Screening executes an initial rapid assessment for drugs of abuse. Although Screening is cost-efficient for eliminating drug use in most samples, a positive screen needs confirmation to be court-admissible. Any sample flagged as presumptively positive in Screening necessitates a secondary confirmation.

If a sample is marked as presumptively positive at the Screening stage, more hair is sourced from the initial sample to prepare for Extraction. At this stage, drugs are isolated from hair at concentrations much lower than in other methods (for example, urine or oral fluid), which is why hair drug testing is the most complex method to conduct.

Confirmation for any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as required. The full laboratory process from Accessioning to Confirmation is evaluated under CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
