For EtG, extra tests, and/or analyses beyond 10 panels, it is advised to use 150mg of the specimen. Using a jeweler's scale to measure the sample is suggested. If scalp hair isn't available, an equivalent quantity of body hair can be used. "Head hair" refers specifically to scalp hair, while "body hair" includes all other varieties (such as facial or underarm hair).

Process Overview

The primary processes in lab analysis of a drug test include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails introducing a sample into the laboratory's procedure. It involves checking the sample's closure and shipment, assigning a unique LAN (Laboratory Accessioning Number), and filling in any additional information not covered by a digital chain of custody system.

Screening involves an initial rapid check for drugs. Although it's cost-effective, positive results from Screening require confirmation to be used in legal proceedings. Samples that test positive initially in Screening need subsequent verification.

If a sample tests positive at the Screening phase, more hair is extracted from the initial sample for Extraction. In this phase, substances are separated from hair at significantly lower concentrations than other methods (like urine or saliva), making hair drug analysis the most complex to conduct.

Positive Screening results are verified using GC/MS, GC/MS/MS, or LC/MS/MS. Any initial positive samples are cleansed before confirmation if necessary. The lab procedure, from Accessioning to Confirmation, undergoes assessment under both the CAP (College of American Pathologists) Hair designation and accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
