A 1.5-inch segment of approximately 200 hair strands (about the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample size for screening and verification. For EtG, add-ons, and/or tests beyond 10 panels, a 150mg sample is suggested. We advise using a jeweler’s scale for weighing the specimen. If head hair is not available, a comparable quantity of body hair can be used. The term "head hair" refers to scalp hair only, while "body hair" pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four core steps in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample within a lab’s system. This process involves ensuring the sample was properly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody system.

Screening conducts an initial quick assessment for drugs of abuse. While Screening is a cost-friendly way to dismiss drug usage in most samples, a positive screen requires confirmation to be valid in court. Any samples deemed presumptively positive during Screening need a subsequent confirmation.

If a sample tests presumptively positive in Screening, more hair is taken from the initial sample for Extraction. During this phase, drugs are extracted from hair at a lower concentration than in other methods (e.g., urine or oral fluid), making hair drug analysis the most challenging methodology to execute.

Confirmation of any positive Screening result uses GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as needed. The complete laboratory workflow from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
