If scalp hair is not available, an equivalent volume of body hair can be used. When mentioning head hair, it pertains exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory methodology for processing a drug test consists of Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a lab’s system. This involves verifying that the sample was adequately sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and handling any additional data entry that may not be covered by an electronic custody system.

Screening incorporates a preliminary rapid inspection for illicit substances. While Screening offers a budget-friendly means to rule out drug use in most samples, a positive screen must be verified for court admission. Any samples showing a presumptive positive in Screening require secondary confirmation.

If a specimen shows a presumptive positive in the Screening phase, additional hair is retrieved from the initial sample and prepared for Extraction. At this phase, drugs are drawn from the hair at significantly lower concentrations than in alternative methods (e.g., urine or oral fluid), which is why hair drug analysis is the most challenging method to perform.

Verification of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before verification as necessary. The full laboratory process from Accessioning to Confirmation undergoes evaluation under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
