For EtG, add-ons, or tests beyond 10 panels, 150mg of the sample is suggested. It is advisable to measure the sample using a jeweler's scale. If scalp hair is not available, a similar quantity of body hair can be taken. "Head hair" pertains solely to scalp hair, while "body hair" includes all other hair varieties (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result involves Accessioning, Screening, Extraction, and Confirmation as the main steps.

Accessioning refers to the first processing stage of a sample in a lab's system. This involves verifying the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and filling out any additional data not available from an electronic chain of custody system.

Screening is an initial prompt examination for drugs of abuse. Though Screening is a cost-effective method to exclude drug use in most samples, a positive screen must be confirmed for court admissibility. Any samples with a presumptive positive in Screening need further confirmation.

Should a sample be presumptively positive at the Screening stage, additional hair is taken from the initial sample and prepped for Extraction. At this stage, drugs are extracted from hair at lower concentrations than those in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation if necessary. The entire lab process from Accessioning to Confirmation is evaluated under CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
