What it is, How it works

Hair testing is increasingly recognized as an effective method for identifying drug and alcohol consumption. Hair captures enduring records of alcohol and drug presence by embedding biomarkers in the strands of hair. When gathered from the scalp, hair can offer approximately a 3-month detection period for alcohol and drugs. Hair collection is straightforward, reasonably resistant to tampering, and uncomplicated to transport.

A 1.5-inch section of about 200 hair strands (comparable to a #2 pencil's thickness) nearest to the scalp provides 100mg of hair, the optimal amount for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is advisable. We suggest measuring the sample with a jeweler's scale. If scalp hair is not available, an equivalent volume of body hair can be used. "Head hair" specifies only scalp hair, whereas "body hair" includes all other types (facial, axillary, etc.).

Process Overview

There are four principal stages in the laboratory analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning signifies the sample's first step into the lab system. The process verifies the sample's proper sealing and shipment, assigns a random LAB Accessioning Number (LAN), and enters any additional information not captured by an electronic custody chain system.

Screening is a preliminary assessment for the presence of drugs. Although Screening offers a cost-efficient means to dismiss drug use in most cases, a positive outcome must be verified to be court-admissible. Samples that show a presumptive positive result in Screening require further confirmation.

If Screening indicates a presumptive positive, additional hair is taken from the initial sample for Extraction. At this stage, drugs are isolated from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), thus making hair drug screening the most challenging method to execute.

Conferring any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing as needed before confirmation. The entire process, from Accessioning to Confirmation, is validated per the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
